Use this service to cancel an appointment. Let us know as soon as possible if you cannot make the appointment.

Online requests submitted before 6:30pm will be acknowledged on the same day (within opening hours).

• are registered at the surgery

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
