Sharing Your Information

Your care service use computer systems which allows GPs, consultants, nurses and other healthcare professionals to record patient information securely.

Care Services can include your GP practice, child health, community services, palliative care, urgent care and more.

It is possible for information to be shared with other healthcare professionals so that everyone caring for the patient is fully aware of the patient care plan, subject to the patient consent. This enables the best possible care to be provided.

The NHS care service uses a secure information sharing system. When you register with the practice you will be given a consent form around sharing of information. If you wish any further information please ask reception.
