In order to assist us in this we require that you take full responsibility to ensure you keep medical appointments and follow the medical advice given.

Occasionally practice/patient relationships break down completely. In this situation the patient may choose to register with another practice. The practice also has the right to remove patients from their list. The patient would be given a specific reason for removal, and it would only occur after a previous warning had failed to remedy the situation.
